We will go through the difficulties and I will highlight small but very important decision making dilemmas that can ultimately be useful tips to help you solve composite restorations like these and others. I am confident that my dear colleagues will be enriched with practical advice that they can put to good use on their next working day.

I would like to present the application of the innovations that I use and consider important in everyday dentistry, combined with the practice and experience of almost 3 decades. The lecture will focus on the presentation and transfer of practical tips and tricks that are easy to learn for everyone and can be applied in everyday dentistry through the presentation of clinical cases.

Treatment of subgingival deep cavitations, lifting of cavities – practical implementation and limitations
In posterior teeth that require root canal treatment, we often encounter deep subgingival caries in the proximal region, the remediation of which is an essential prerequisite for endodontic treatment of the tooth. Restoration of the resulting cavity, which has already been cleaned, often creates a difficult situation in clinical practice. This is often treated as an “endodontic stump reconstruction”, which may imply a temporary nature, although this is completely wrong. The current, modern, English nomenclature, i.e. “Deep Margin Elevation (DME)”, reflects much better both the purpose and the difficulty of the procedure. DME is defined as an adjunctive intervention in the most difficult circumstances. It also determines the success of a full, permanent restoration and is a key factor in the process of deciding whether a tooth is retained: “if we cannot do it properly, how do we want to treat and retain the tooth now or in the future?”. In my presentation, I would like to present a modern, easy-to-implement protocol based on current literature and some tricks that can make the restoration of these severely destroyed teeth easier and more predictable.

It’s a long way from cracked tooth to fracture. In my presentation I will try to cover this path in depth, from all angles, through cases that every dentist encounters every day.
I would like to present the biomimetic approach which, among other things, tries to avoid the formation of a crack, but also offers solutions to existing problems, be it a crack or a chipped tooth.

Biological Treatment of Teeth – Almost All Indications of Bioactive Cements
When preparing a deep cavity, do you wonder how deep you can drill? How much demineralized dentin can you safely remove? Do you have concerns that you will expose the pulp and must perform root canal treatment? Or maybe you want to increase the effectiveness of your root canal treatment? Do you want to see if complications such as canal perforation are treatable?
If so, I invite you to my lecture, where I will talk about the surprising properties of bioactive cements in the treatment of caries, dental pulp, and periapical tissues. You will see how to use Bioroot RCS and Biodentine in daily practice. You will learn which of these materials to choose in a specific situation. I will show you simple procedures for the treatment of deep caries, single-visit root canal treatment as well as treatment of perforation and wide physiological foramen in the root canals.

Aesthetic tooth replacement as if nothing had happened
Patients’ expectations of the aesthetic results of dental treatment are higher than ever. What’s more, they expect to achieve this with minimal intervention and maximum tooth preservation. But is it really possible to restore teeth as if nothing had happened to them? Are there limits to what can be corrected and what is irreversibly lost?
Many events can damage the aesthetics of a smile, so we dentists have to be creative in our treatment solutions.
In some cases, whitening and a simple filling done well may be the best solution. I will talk about the paradigm shift in colour matching for direct restorative treatments.
If there are discoloured teeth in the anterior region, we must not forget the phenomenon of the “umbrella effect”. This affects both pink and white aesthetics, so we will delve deeper into this concept.
Restoring anterior teeth with implants is the new trend in our practice. We will also talk about how to fit natural teeth, veneers and crowns on implants.
Other cases require full mouth rehabilitation with an interdisciplinary approach including orthodontics, implants, tissue grafts, crowns and veneers. Even if the specialties are different, the goal is the same: to restore a beautiful smile!

Root canal restoration – where is the line between direct and indirect solutions?
Strengthening root canal teeth has long been a coveted goal among practicing dentists. Unfortunately, due to inappropriate restorative solutions, incorrect indication determination, we quite often encounter the fracture of root canal treated teeth, which necessitates tooth removal. This also shows that the restorative solution chosen for these teeth must strengthen the tooth and keep it intact. For this reason, the choice of an indirect restoration may be obvious. At the same time, modern fibreglass materials have now extended their field of application to the indication area of indirect restorations. But is it important exactly which material is chosen or, if modern fibreglass is used, can anything be done with direct techniques? The lecture will highlight the latest indications in the field of choice of direct and indirect restoratives for root canal treated teeth and will also present the exact application protocols.

Rubber dam: enemy or friend?
In my opinion, absolute isolation is an indispensable requirement for modern dentistry. This form of isolation can help in both endodontics and restorative dentistry due to its many advantages.
The main aim of my lecture is to encourage the use of the cofferdam. I also want to reassure those who use it but are unsure. I will also provide tips and tricks for fanatics like me, making it easier to isolate the more difficult cases.

An update on targeted endodontic microsurgery
With the help of modern diagnostic methods and advanced treatment options in dentistry, the success rate of endodontic treatments rose steeply in the last decades. If the orthograde approach does not eliminate the occurring symptoms, in the presence of a lege artis root canal obturation, surgical removal of the apex and the inflamed periapical tissues is used as a routine intervention. Innovations of implant dentistry and the presence of CBCT can offer a combined method to reduce the steps and aid the healing after apicoectomy. The unique visualisation provided by the CBCT and numerous planning software can improve the planning phase and precision of any kind of surgery. These methods are wide spread in implant dentistry nowadays. Hence the aim is the same, 3D planning and 3D printed surgical guides can be used also on other fields of dentistry. On the other hand, the necessity of root end surgery is inevitable in certain cases. Because of the documented wide range success rates of early surgical endodontic treatments (44-95%), the remodelling of apical surgery principles, with the presence of surgical microscopes was a relevant issue. The minimal invasive surgical therapy was introduced and described as a basic method by Kim S. et al. and since then the revolution of dental surgery has started. This inspired us to find and prove this novel method, combining 3D printed templates and trephines for apicoectomy. The presentation will first review the latest innovations in the field of targeted endodontic microsurgery starting from the first preclinical research in the field in 2007. The evolution of the technique will be discussed, so as the innovative steps which has led to the currently emphasized fully guided endodontic surgery with a trephine but. The relevant preclinical and clinical experiments of our research group will also be introduced, showing the background of the technique, that has now been described in numerous case-reports, but only in a few real scientific studies.

I believe that the key to successful treatment is not artistic skill, but thoughtful planning and precise execution.
